Equipment Failure Simulator

Premium tool. This is an embedded simulator inside the Blood Transfusion Virtual Lab — open /virtual-laboratory, choose Blood Transfusion, then trigger an equipment-failure scenario inside the lab. It is not a standalone entry on the /training-dashboard hub.

When an analyser fails — especially out of hours — the BMS on duty has minutes to make decisions that affect patient care. This simulator drills the contingency workflow.

Decision framework

The simulator teaches a structured response:

  1. Stabilise — pause new sample acceptance for affected analytes; quarantine in-progress samples
  2. Triage — what work is truly urgent (active resuscitation, MHP, transplant) vs deferrable?
  3. Backup route — manual method, neighbouring lab, send-out to a regional reference lab
  4. Communicate — clinical teams, line manager, on-call consultant, vendor
  5. Document — every decision time-stamped; produces a contingency-incident log
  6. Recover — fix, retro QC, validate first samples on the restored analyser before resuming normal operation
  7. Review — RCA the failure (see article 48), update the contingency plan
