Equipment Failure Simulator
Premium tool. This is an embedded simulator inside the Blood Transfusion Virtual Lab — open /virtual-laboratory, choose Blood Transfusion, then trigger an equipment-failure scenario inside the lab. It is not a standalone entry on the /training-dashboard hub.
When an analyser fails — especially out of hours — the BMS on duty has minutes to make decisions that affect patient care. This simulator drills the contingency workflow.
What it does
- Presents a critical analyser failure scenario (mid-day, on-call, mid-MHP)
- Walks you through immediate-response priorities
- Tests your backup workflow decisions — manual methods, neighbouring lab support, sample diversion
- Drills contingency communication — clinical teams, line manager, vendor
- Handles recovery — return-to-service criteria, retro QC, validation of in-progress samples

Decision framework
The simulator teaches a structured response:
- Stabilise — pause new sample acceptance for affected analytes; quarantine in-progress samples
- Triage — what work is truly urgent (active resuscitation, MHP, transplant) vs deferrable?
- Backup route — manual method, neighbouring lab, send-out to a regional reference lab
- Communicate — clinical teams, line manager, on-call consultant, vendor
- Document — every decision time-stamped; produces a contingency-incident log
- Recover — fix, retro QC, validate first samples on the restored analyser before resuming normal operation
- Review — RCA the failure (see article 48), update the contingency plan
Backup workflow examples
- Haematology — manual differential by film microscopy; neighbouring analyser
- Biochemistry — POCT for K, glucose, ABG; send-out for less urgent
- Coagulation — manual PT/APTT (Lee-White method historic), or send to nearest hub
- Blood gas — POCT alternative if available
- Histology — manual processing of urgent fresh tissue; defer routine until recovery

- ISO 15189:2022 clause 6.6 (Equipment) and continuity-of-service expectations
- MHRA vigilance reporting if device-defect contributed
- Trust major-incident plan — analyser failure can escalate to major incident
- UKAS has explicit expectations on contingency planning
